Problem
This article goes over how to recreate the default SOLIDWORKS Part, Assembly, and Drawing Templates in cases where they are missing or no longer available.
Solution
To have SolidWorks regenerate the default templates:

Browse to Tools > Options > File Locations and make sure the drop-down bar says "Document Templates"
- Navigate to these locations within Windows Explorer and rename the folder(s) containing the template files (.prtdot, .asmdot, .drwdot).
- After restarting SOLIDWORKS, click File > New, and the new template files will be generated.
